Due to severe
shortage of Family Physicians we accept a
limited number of patients into our Family Practice group.
Our Flexible Access
Care model will provide the medical care you need at the time you
need it. You will not be required to wait for days to see a
doctor. You will have 7 day a week walk-in access to our Family
Practice group at our Niagara Falls Clinic. Please
talk to our receptionist when you come to see if we are currently
accepting applications.
For continuity of
care reasons, patients accepted into the Family Practice will be
expected to receive all their primary medical care at the UCN clinic they register with. We encourage you to
continue to use the Hospital Emergency Room for life or limb
threatening emergencies.

Will I see the
same doctor on each visit?
With the Flexible
Access Care approach to Family Practice you will be provided care by
all the physicians in our group. All the physicians will work with
and have access to your entire medical chart at the clinic. You
will be able to see the physician(s) who is scheduled to work at the
time of your visit. We feel with the physician group approach this
will provide continuity of care for you even if one of our
physicians is on vacation, retires or moves to another city.
Will I have to
wait for days or even weeks to book an
appointment to see a doctor?
NO! Rather than wait for days to get
an appointment for an acute illness, we feel with Flexible Access
Care you can decide when you need medical care. We will
continue to provide care on a walk-in basis. Our job as
physicians is to do our best to see you the same day and on a timely
manner. Please note that no medications will be renewed on the
phone. We will be happy to renew your medications during your
regular visits.
Can I get my
medical records
transferred from my previous doctor?
It is important to
have access to all relevant past medical records.
We are on a state of the art fully computerized medical record
system. Your records generated after each visit with us will be on
your computerized chart at our clinic. These records can be
accessed or printed out for any of your visits or to refer you to a
specialist as needed.
We will facilitate
transfer of your records from your previous physician. Once
received you will be the custodian of your old records and when
needed you will be asked by one of the physicians to bring your old
medical records to the clinic.
